Paul Smith

With an extensive background in nursing and medical and geriatric case management, Paul R. Smith R.N. has led Extended Family Associates in San Luis Obispo, California, since 2007. The firm takes pride in enabling elderly and disabled individuals to stay at home and maintain maximum personal independence. Paul R. Smith R.N. coordinates physical care with often geographically dispersed family members, as well as with caregivers, physicians, accountants, and attorneys as necessary.

Earning his Bachelor’s degree in History and Political Science at Marquette University in Milwaukee, Mr. Smith completed his B.S.N. at Omaha’s Creighton University through the institution’s accelerated nursing program. He subsequently received his Human Resource Management M.B.A. from the Keller Graduate School of Management.

In addition to his current position, Mr. Smith served as Case Manager with Gentiva Health Services in San Luis Obispo from 2010 to 2012. He performed assessments of up to15 homebound patients, created and evaluated care plans, and provided direct nursing services. He also supervised the work of assistant team members and coordinated care with relatives, nursing management, and a diversity of speech, occupational, and physical therapists.

A primary influence on Paul R. Smith R.N.’s dedication to helping others has been his experience assisting his fiancée when she received a cancer diagnosis. He provided support through the process of treatment and recovery, including radiation, surgery, and chemotherapy. Mr. Smith also took care of his mother in her last months, striving to make her life at home as free of restrictions as possible. For the past four years, Paul R. Smith R.N. has maintained membership in the National Association of Professional Geriatric Care Managers.
